Schools can use the forms below to refer cases to the Attendance Support Team for consideration and action. Please ensure you have reviewed the relevant guidance before submitting a referral and that all supporting evidence is included where required.
Available Referral Forms
Term-Time Holiday and Penalty Notice Referrals
For requests relating to:
- Unauthorised leave of absence taken during term time that may meet the criteria for a Penalty Notice.
- Cases where a Penalty Notice has been agreed as the appropriate outcome following Legal Intervention Triage for persistent absence or persistent lateness.
Before submitting
Please ensure that:
- The attendance records and absence codes have been completed accurately.
- The case meets the current Penalty Notice thresholds and requirements.
- All relevant attendance, absence and supporting evidence has been gathered.
- Where applicable, the case has been discussed with the school’s allocated Education Welfare Officer and considered through the Legal Intervention Triage process.
- Details of any Legal Intervention Triage meeting and agreed outcome are available for inclusion within the referral.

Truancy Penalty Notices
For cases where a child has been identified during a truancy patrol and the matter is being referred for consideration of a Penalty Notice.
Before submitting:
- Confirm the pupil was absent without authorisation at the time of the patrol.
- Ensure all requested information is available.

Legal Interventions
For attendance cases that may require statutory intervention where voluntary and formal support has not achieved the required improvement.
This referral may lead to:
- Attendance contracts
- Education Supervision Orders
- Prosecution consideration
- Other legal attendance interventions

Complex Attendance Panel
For pupils with persistent or severe attendance concerns where a multi-agency discussion is required to consider barriers, support and next steps.
Suitable referrals may include:
- Long-standing attendance concerns
- Multiple unmet needs
- Cases requiring coordinated agency involvement
- Situations where previous interventions have not secured improvement
