Child Performance Licence application form

We’re pleased to introduce our new online submission form for performance licence applications. This streamlined process is designed to make it quicker and easier for you to submit the necessary details for children taking part in performances, rehearsals, or activities covered by licensing regulations.

Supporting documentation

Supporting documentation may be required depending on the nature of the application. Please make sure these are available to you whilst completing this new application. Required documents may include:

A copy of the child or young person’s birth certificate or satisfactory evidence of child’s age
Two passport sized photographs taken within the previous six months
Risk assessment – for location and relevant to the child
Child Protection Policy
A letter from the child’s school stating absence is permissible (if absence is required for the performance). We must be satisfied that arrangement for the supervision and protection of the child are adequate and that the disruption to the child’s education is kept to a minimum.
A downloaded medical declaration form completed and signed by the parent/ guardian (to be uploaded when prompted).

Please ensure all required information is completed accurately to avoid delays in processing, a copy of this application will be sent to the email address you provide at the top of the application.

“Night work” refers to any performance that takes place during the late evening or night. Specifically, the regulations define the latest permitted hours for children as follows: Children under 5 years old: Up to 10 PM. Children aged 5 and over: Up to 11 PM.
