To complete this application form you will need to the following:
- You need to be a Sutton resident (chaperones are licenced by their home boroughs)
- Enhanced (child workforce) DBS certificate. We require all chaperones to carry a valid DBS certificate, if you have this already on the DBS Update Service you will be asked for these details, if you are not registered we will facilitate a new application for you to complete as part of the process.
- A recent photograph for your licence, you should be fully facing the camera, this should have a light background, however feel free to smile!
- 2 referee’s and their contact details (Who CANNOT act as a referee: A family member/ partner/ partner’s family; A person involved in any production in which you will/ could be acting as a chaperone; Another registered chaperone)
CHILDREN AND YOUNG PERSONS ACT 1963 The Children (Performance and Activities) (England) Regulations 2014.
The licensing authority must not approve a person as a chaperone unless it is satisfied that the person—
(a) is suitable and competent to exercise proper care and control of a child of the age and sex of the child in question; and
(b) will not be prevented from carrying out duties towards the child by duties towards other children
Regulation 15(4) (a) and (b) The Children (Performance and Activities) (England) Regulations 2014 “Any person who knowingly or recklessly makes any false statement in or in connection with an application for a licence…. shall be liable on summary conviction to a fine not exceeding £1,000, or imprisonment for a term not exceeding three months or both” (Children and Young Persons Act, 1963, Part 11, Section 40)
