Who needs to apply for a work permit?
The employer should apply to the Child Employment and Performance Licensing Officer for a permit if the child will be working in the London Borough of Sutton. A permit is for a specific job and cannot be transferred.
What you will need to complete an application:
- Childs and parent details
- Information on the role including proposed working days and times
- A recent photograph of the child (within 6 months) (to be attached within)
- A complete medical declaration form, filled out and signed by the parent/ guardian (to be attached within)
- A safeguarding / child protection policy and a work place risk assessment (or a combined safeguarding and work place risk assessment document)(to be attached within)
